1. Arrive 10 minutes earlier to fill out the chart
2. Bring all your insurance information and benefit booklet (if available). We will accept your dental insurance plans, bill your insurance directly and electronically. We will assist you in understanding your dental benefits.
3. Please bring a list of any medications you are taking.
4. We require 48 hours notice for shedule changes on weekday appointments and 1 weeks notice for schedule changes on Saturday appointments.
